While replacing the seal can often resolve the problem, if the seal keeps falling off and rattling around the slip yoke, it may be a sign of a worn or damaged output shaft or yoke. The output shaft seal is to prevent fluid leakage from the transmission or transfer case and to keep dirt and debris out of the system. If the output shaft or yoke is worn or damaged, it can cause the seal to fail prematurely or not stay in place.
